REGISTERING A BUSINESS NAME

WHO CAN APPLY?

  • Filipino Citizen
  • 18 years old and over
  • Single/sole proprietor

WHAT ARE THE REQUIREMENTS?

Single Proprietorship

1) Registration fee of Php 300.00
2) Three (3) alternative names aside from the preferred name
3) Three (3) pieces documentary stamps
4) Application forms duly filled-up and signed by owner

PROCESSING FEES:

  • Php 300.00 – Sole proprietorship ( New, Renewal and Change of Business Name)
  • Php 50.00 – Change of information
  • Php 15.00 – Documentary stamp

For Service and Repair business, DTI accreditation is required

FOR RENEWALS:

Completely filled up RENEWAL Application Form (Only the Owner of the business is authorized to sign all the forms)

Bring original copy of old Business Name (BN) Certificate and duplicate copy of the Application form. Submit an Affidavit of Loss if these documents were lost

Additional requirements on a case basis depending on the actual examination & processing of the application

FOR CHANGE OF BUSINESS NAME:

Completely filled up NEW Application Form. (Only the Owner of the business is authorized to sign all the forms)

Notarized Affidavit of Cancellation of the registered BN, stating the reasons for the cancellation and that the registered Owner has no financial obligation

For Single Proprietorship

HOW TO APPLY FOR A MAYORS PERMIT

APPLYING FOR A MAYOR’S PERMIT FOR NEW BUSINESSES

ABOUT THE SERVICE

All business undertakings/activities conducted within the territorial jurisdiction each City are mandated by national laws and local ordinances to secure the necessary Permit to Operate and pay the corresponding taxes and regulatory fees before commencing operation.

The Mayor’s Permit must then be subsequently renewed annually before January 20.  Renewals made after the afore-said period are subjected to interests and penalties.

FEES

Taxpayers applying for a Mayor’s Permit for a new business shall pay regulatory fees based on the nature of business and the amount of its capitalization.

GENERAL REQUIREMENTS:

  1. Barangay Clearance
  2. Proof of payment of real property taxes if the place of business is owned by the applicant; for lessees, a contract of lease can be submitted in lieu of this requirement.
  3. DTI Certificate/SEC Registration, whichever is applicable
  4. Copy of public liability insurance secured from accredited insurers
  5. Occupancy Permit of the building where the business will be established
  6. For Cooperatives: Cooperative Dev. Authorizes, Registration, General Information Sheet.

ADDITIONAL REQUIREMENTS:

  • For those transacting through representatives, the ID of the owner, together with an authorization letter naming the representative must also be presented.
  • For restaurants and other businesses involved with food and/or handling, a copy of the Sanitation Permit secured from the City Health Office.
  • For businesses that are tourism-related, such as spas, hotels, motels, travel agencies, tour guides, and money changers, a copy of the Tourism Certificate issued by the
    City Tourism Office.
  • For businesses that involve animals, (eg. Pet Shop, slaughterhouse, etc.,) Veterinary Clearance
  • For renewal of permits: Previous Mayor’s Permit and Latest Official Receipt

RENEWING A MAYOR’S BUSINESS PERMIT

FEES

Taxpayers applying for the renewal of their Mayor’s Permit shall pay business taxes and regulatory fee based on the nature of business and the amount of its gross sales for the
succeeding year.

REQUIREMENTS:

Under the “automatic renewal” scheme of the City, a business permit is renewed without the need for other requirements than the permit itself and payment of the necessary fees,
provided that it is not in the “hot list” of erring businesses as may be prepared by concerned departments such as the fire, engineering, etc.

APPLYING FOR A WORKING PERMIT (NEW & RENEWAL)

ABOUT THE SERVICE

All employees working within the territorial jurisdiction of each City are mandated by laws and local ordinances to secure the necessary Work Permit and to pay the corresponding fees therefore.

The Work Permit must then be renewed annually.

FEES

Work Permit applicants shall pay fees based on the position being applied for.

  2. Receive the BIR 0605 Payment Form
